Online Consent Forms for School Flu Vaccination Program Now Available

The Georgia Department of Public Health’s South Health District is excited to announce that flu vaccination consent forms for students are now available online for the 2024-2025 flu season.

Each year, as part of our ongoing commitment to making healthcare more accessible, health departments across the district will be visiting local schools to administer flu vaccines to students. However, this year, our district wants to make that process even easier for parents by offering the consent forms online.

By offering these forms online, we aim to streamline the process and make it more convenient for parents to ensure their children are protected against the flu this season. Parents can now easily access and complete the consent forms from the comfort of their own homes.

Flu vaccines will only be administered to children whose parents or guardians have completed the consent forms.
