| SCOPE OF WORK |
| Provide leadership training as stated in attachment A for South Health District supervisors and leaders. Commercial General Liability Insurance of $1,000,000 per occurrence; $2,000,000 general aggregate. Professional Liability (Errors and Omission): Coverage appropriate for the services being provided under this contract. Worker’s Compensation: Coverage as required by the laws of the State of Georgia. South Health District reserves the right to request evidence of coverage prior to contract execution and throughout the term of the agreement. | Deliverables to be provided by Party A: |
| Deliver an on-going comprehensive leadership program tailored to South Health District for current and emerging leaders to strengthen leadership capabilities. Provide a structured program for small groups (20-25 participants) in no more than ten face-to-face workshops within six months. Cover topics such as: • Workplace ethics and law. • Communication styles and strategies. • Leadership styles. • Conflict resolution and difficult conversations. • Providing effective feedback. • Team development and team building. • Engagement and retention strategies. • Workplace violence. • Employee burn out and mental health. • Leadership effectiveness. Provide and/or create program curriculum and meet face-to-face at least 90% of workshops. An estimate of all travel and supply costs should be included in proposal. Party A shall assume full responsibility and liability for himself/herself and his/her employees while on the premises and shall hold the South Health District harmless for any injury or damages caused by their negligence. Please attach a copy of $1,000,000 personal injury insurance and $1,000,000 general liability for each occurrence/$2,000,000 aggregate, business license, and a completed W9. |
