Please Note: Insurance may cover some services. If you are uninsured or your insurance does not cover a specific service, fees will apply for all clinic services.
Back-to-School Immunization Requirements
Georgia law requires students to be up to date on certain vaccines before starting school. Immunizations help protect your child, their classmates, and the entire community from serious diseases.
Vaccines your child may need include:
- Tdap (Tetanus, Diphtheria, Pertussis)
- Human Papillomavirus (HPV) series
- Chickenpox (Varicella)
- MMR (Measles, Mumps, Rubella)
- Hepatitis A
- Hepatitis B
- MCV4 (Meningococcal Conjugate Vaccine)
Important Grade-Specific Vaccine Reminders
- 7th Grade:
- One dose of meningococcal (MCV4) vaccine
- One dose of Tdap (whooping cough booster)
- 11th Grade:
- A meningococcal booster (MCV4) is required unless the first dose was given on or after the 16th birthday.
Note: MCV4 helps protect against four types of bacteria that cause meningococcal disease. Most people experience only mild side effects, such as soreness where the shot was given or a low-grade fever, which typically goes away within 1-2 days.
Required Health Forms
In addition to vaccines, students entering a Georgia school for the first time (any grade) must have:
- Form 3231 (Georgia Immunization Certificate)
- Form 3300 (Ear, Eye, Dental, and Nutrition Screening)
These forms are available at your local health department and must be completed before school entry.
