As Pre-K and Kindergarten registration begins in several counties, South Health District reminds families that your local health departments are available to help you complete required school health forms and services.

Health departments can assist parents and guardians with:

• Immunizations (Form 3231)
• EEDN (Eye, Ear, Dental and Nutrition) screenings (Form 3300)
• Certified birth certificates, available at the Brooks, Echols, Lowndes, and Tift County Health Departments

Families are encouraged to call or visit their local health department to schedule an appointment.
